"Integrated cardiac and metabolic care — assessing the heart and the conditions that put it at risk, together."

Cardiovascular disease and metabolic conditions are deeply interconnected. Patients with type 2 diabetes face up to four times the cardiovascular risk of those without. Obesity drives hypertension, dyslipidaemia, and structural heart changes. Treating these in isolation leads to incomplete management.

Our integrated cardiology and endocrinology model — a distinctive integrated cardiometabolic service for patients in Naas, Kildare and the wider Leinster region — allows us to assess and manage these overlapping risks in a single clinic visit, with a coordinated treatment plan and unified correspondence to your GP.

Your Cardiologist

Dr Imtiaz Ali Kalyar is a Consultant Interventional Cardiologist with 16 years of interventional cardiology experience, including over 9 years at full consultant level across NHS trusts in the UK. He completed his Interventional Cardiology Fellowship at Queen Alexandra Hospital, Portsmouth — one of the UK's leading tertiary cardiac centres — and has performed over 1,459 Percutaneous Coronary Interventions (PCIs) as first operator, including 406 Primary PCIs for acute myocardial infarction.

Dr Kalyar is an independent operator for Chronic Total Occlusion (CTO) PCI, Rotational Atherectomy, and Intravascular Lithotripsy (IVL). He performs IVUS, FFR, iFR/rFr and OCT independently. He has been Principal Investigator for multiple landmark clinical trials including SENIOR-RITA (published in the New England Journal of Medicine, September 2024), BLADE, and DUAL ACS.

His partnership with Dr Kazmi creates a distinctive integrated cardiometabolic service — enabling holistic, one-stop management of patients with overlapping diabetes, obesity, and cardiovascular disease, of particular value for GPs managing complex cardiometabolic patients.

Interventional Cardiology

Dr Kalyar is a Consultant Interventional Cardiologist with one of the most extensive procedural portfolios available in private practice in Ireland. His interventional experience spans elective and emergency coronary intervention, complex lesion subsets, and advanced intracoronary imaging and physiology.

Percutaneous Coronary Intervention (PCI)

Over 1,459 PCIs performed as first operator including 406 Primary PCIs for acute ST-elevation myocardial infarction (STEMI). Extensive experience with elective, urgent, and emergency coronary intervention via both radial and femoral approaches.

Chronic Total Occlusion (CTO) PCI

Independent antegrade CTO operator — one of the most technically demanding procedures in interventional cardiology, requiring advanced wire-escalation and microcatheter techniques to reopen long-standing coronary artery occlusions.

Rotational Atherectomy

Independent Rotational Atherectomy operator for heavily calcified coronary lesions that are resistant to standard balloon preparation — enabling PCI in patients who would otherwise be unsuitable for percutaneous intervention.

Intravascular Lithotripsy (IVL)

Independent IVL operator — a newer modality using sonic pressure waves to fracture severe coronary calcification, facilitating stent delivery and expansion in complex calcified disease without the risks of high-pressure atherectomy.

Intracoronary Imaging & Physiology

Independent operator for IVUS (intravascular ultrasound), OCT (optical coherence tomography), FFR (fractional flow reserve), iFR and rFR — enabling precise lesion assessment, stent sizing, optimisation and post-dilatation guidance for the best possible outcomes.

Primary PCI On-Call & Acute Cardiac Care

Extensive experience on Primary PCI 24/7 on-call rotas at multiple UK NHS centres. CCU ward rounds, cardiology consults, A&E triage, and management of acute coronary syndromes, arrhythmias and haemodynamic instability.
