How to Refer

Our referral process is straightforward. Healthmail is the preferred method for sending patient-identifiable clinical information securely.

Send Your Referral

Send a referral letter via Healthmail to Syed.Kazmi@healthmail.ie. Include a brief clinical summary, current medications, and relevant investigation results. Telephone referrals accepted on 089 656 7597.

Acknowledgement

You will receive acknowledgement within 2 working days. We will contact your patient directly to arrange an appointment at a mutually convenient time. Sundays 9am–5pm, Vista Primary Care Clinic, Naas.

Consultant Assessment

Your patient receives a thorough consultant-delivered assessment. Relevant investigations may be arranged on the day. A same-sex chaperone is available on request — please indicate in your referral if required.

Clinic Letter to You

A structured clinic letter is sent promptly following consultation, summarising the assessment, investigations, diagnosis, and management plan. Results and investigation reports are copied to the referring clinician.

Referral Acknowledged

We aim to acknowledge all referrals within 2 working days of receipt. If additional clinical information is required, we will contact you promptly via Healthmail or telephone.

Patient Contacted for Appointment

Your patient will be contacted directly to arrange a convenient appointment. Clinic operates on Sundays 9am–5pm at Suite 5, Vista Primary Care Clinic, Ballymore Road, Naas, Co. Kildare.

Comprehensive Consultant Assessment

Your patient receives a thorough consultant-delivered assessment. Relevant investigations may be arranged at the time of consultation. A same-sex chaperone is available on request.

Clinic Letter Returned to You

A comprehensive clinic letter is sent to the referring clinician following consultation, summarising the assessment findings, investigation results, diagnosis, and management plan. Copies are provided to any additional named clinicians as appropriate.

⚡  Urgent Referrals: For clinically urgent cases requiring an expedited appointment, please telephone 089 656 7597 or send a priority-flagged Healthmail to Syed.Kazmi@healthmail.ie with "URGENT" in the subject line. For life-threatening emergencies, please direct the patient to the Emergency Department or call 999 / 112.
